Watch for it! The first Congregational Health Needs Survey will be presented to the MVLC congregation on Sunday 08 October, following each service. Everyone age 13 and up will be provided a survey. We ask that you fill it out immediately following the worship service, without exiting. Completed surveys will be collected by a Faith Community Nurse or helper. Then, off to coffee you can go!
Your Health Ministry team needs your input to primarily gain key information from those we serve, the congregants, on what you perceive are your health needs—in body, mind, and spirit.
It is our hope, as your Health Ministry Team, to use the information we gather to look at programs we can tailor just for our congregation. Other Faith Community Nurses within Pierce and King County will administer this survey to their congregants. The information collected will help the Faith Community Nursing/Health Ministries Northwest look at the needs and how to support church health ministries teams in the work they do.
This survey is simple and short. You will not put your name on it. If you have individual concerns or questions, then please speak with one of the Faith Community Nurses directly. We will be happy to address your concerns and/or answer your questions.
We thank you so much for your participation!
